• APPOINTMENT SHEET Issued by the National Visa Center or the American Consulate.
• MEDICAL EXAM You must present it on the day of your interview and if the Medical Panel is going to send it directly to the Consular Section, you must confirm with them that the results will be ready by that date. If they will not be ready you must reschedule a new appointment.

• PASSPORTS
• PHOTOS: You must bring two color photos with a white background, size 2 X 2 inches, (51 x 51 mm).
• DS-260 THE CONFIRMATION SHEET. Remember that the information on the form must be exactly the same as in the passport.
• ROLE OF GOOD CONDUCT: Also, if you have been arrested, please bring all documentation regarding your arrest.
• ROLE OF GOOD CONDUCT FROM ANOTHER COUNTRY: If you have resided in any other country for 6 months or more since the age of 18, you must present the certificate from the police of that country.
• INEXTENSE LEGALIZED BIRTH CERTIFICATE
• LEGALIZED INEXTENSIVE MARRIAGE CERTIFICATE
• CERTIFICATE OF DIVORCE OR DEATH OF YOUR SPOUSE
• AFFIDAVIT INCLUDING FORMS 1040 AND W2 FOR THE LAST 3 YEARS LAST YEAR OF THE GUARANTOR
• PROOF OF YOUR RELATIONSHIP AS PHOTOS
• OTHER NECESSARY DOCUMENTS:

If the crime occurred in the Dominican Republic, the following documents:

IN THE PUBLIC MINISTRY

1. Certification of non-submission from the Public Ministry / Prosecutor's Office of the Judicial District corresponding to the arrest, indicating that no criminal submission was found against them for that fact. This certificate must specify that no record was found from the date of your arrest until one (1) year after this. In case you have been arrested on more than one occasion, you must present a certificate of non-submission for each arrest.

IN THE NATIONAL DIRECTORATE OF PRISONS

2. Certification from the National Directorate of Prisons stating that you were never confined in any of the prisons controlled by them while serving a sentence.

IN THE DICRIM

3. Certification of fingerprint analysis from the Central Directorate of Criminal Investigations. This documentation must have its translation into English.

THIS IS DONE WITH A LAWYER WHO KNOWS THE AREA

4. A notarized sworn statement signed by you, explaining your version of events. This statement must include the reason for which you were arrested, the date and place of the arrest (s). This statement must have its translation into English.
